Featured Case 02/21: CHIN IMPLANT AUGMENTATION
Patient:
Emma is a 16-year-old female.
The problem:
- Emma has always been very concerned about her profile.
- She does not like her retrusive and small chin.
- She would like a more defined jawline.
- She reported having a large overjet (horizontal space between the upper and lower front teeth).
Patient History:
- Her orthodontist told her he could correct her bite with braces, but her profile and overjet may require jaw surgery.
- She avoided corrective jaw surgery (orthognathic surgery) and completed her braces by age 14.
- She requested to improve her profile, chin projection, and jawline.

- Facial profile analysis has always captured the attention of patients and practitioners.
- Some of the earliest formal facial and profile analyses were developed by the famous Renaissance polymath Leonardo DaVinci.
- Assessment of facial proportions, form, harmony, and profile aesthetics is both a science and an art.
- Oral and maxillofacial surgeons are experts in facial analysis and cosmetic/reconstructive surgery.
- The process of analysis includes a clinical exam, facial photographs, and 3-dimensional imaging.
- The diagnostic measurement and analysis of the facial skeleton is known as cephalometric analysis.
- Varying measurements can be performed for the profile and chin.
- There are ethnic and socio-cultural norms that play a role in the art of analysis and treatment thereof.
- Corrective surgery may entail 2 basic approaches:
- Surgically reposition the jaw and chin (orthognathic surgery). This is best performed in conjunction with orthodontics.
- Implant augmentation
Both approaches have pros and cons depending on the diagnosis and need. The formal term for cosmetic chin surgery is known as Genioplasty.
Procedures:
- Emma had an implant augmentation surgery to place an Implantech extended anatomical contoured chin implant.
- The surgery was performed under IV sedation to ensure she was relaxed, comfortable, and a quick recovery.
- The implant was placed via a small incision in the mouth so no visible scar was evident
Commentary:
- This is a relatively minor procedure with significant results.
- The recovery has some minor swelling for 5-7 days with minimal discomfort.
- The most impactful aspect of this surgery may very well be the confidence instilled in Emma’s demeanor.
